● There was buildup inside a soda gun and its holder.

● Raw chicken was left thawing in a sink.

● Dishwashing machine rinse temperature was inadequate to sanitize dishes and utensils.

● Shredded cheese was not held at the proper temperature.

Rally’s, 1647 W. Laskey, inspected March 2.  

● Food employee(s) did not wash hands in situations that specifically require them to do so.

● Vitamix mixer was left dirty,

● Ice scoop was sitting on dirty gloves and Febreze.

● Clean containers had stickers and sticker residue.

● Hamburger freezer needed defrosting and cleaning.

● Grill cleaner was on top of soda boxes.

Park Inn Toledo, 101 N. Summit, inspected March 2.  

● Storage containers of food were not properly date marked.

● Sanitizing solution in a dishwashing machine did not meet the minimum requirements.

● No sanitizer bucket was set up at the cook line.

● There was buildup on an ice machine shield.

● There was buildup inside an ice machine chute.

● There was buildup of dirt and debris inside soda nozzles.

Shorty’s Back 40, 5215 Monroe, inspected March 6.  

● Person-in-charge did not ensure that employees are properly informed of their duty to report information about their health as it relates to food-borne illness.

Mr. Hero, 5001 Monroe, inspected March 6.

● No person-in-charge was present during inspection.

McDonald’s, 1225 Collingwood, inspected March 6.

● The person-in-charge did not ensure that employees did not touch ready-to-eat foods with their bare hands.

● Staff members were washing hands with finger and wrist jewelry.

● Staff members were contaminating their hands from the floor and not washing hands.

● Clean hands were being contaminated by dirty faucet handles.

● About two pounds of cut lettuce and ranch dressing were held at improper temperatures.

● Ice maker was dirty.

● The ice maker’s scoops rested on dirty surfaces.

● A five-gallon ice bucket sat directly on a dirty floor.

● Air gap between a plumbing fixture and a piece of equipment was insufficient to prevent back siphonage of contaminated water.

Hot Box Bistro, 332 N. Erie, inspected March 6.  

● Raw shell eggs were stored above packaged salmon and other items.

● There was buildup on a potato slicer.

● Hand-washing sink was being used for purposes other than hand washing.

● Food was held at improper temperature.

● There was buildup on soda nozzles.

Wish Upon A Star Learning Center, 5142 Ryan, inspected March 8.  

● No person-in-charge was present in the food facility.

● Person-in-charge did not ensure that employees are properly informed of their duty to report information about their health as it relates to food-borne illness.

● Open hot dogs, bologna, and beans lacked date marks.

● Chemical containers lacked proper labels.

● Cut lettuce was kept past its discard date.

● Sanitizing solution in a dishwashing machine did not meet the minimum requirements.

● There was buildup inside an ice machine and the juice gun nozzle.

● Chemicals were stored on the same shelf as food equipment.

● Sanitizer bucket didn’t meet minimum requirements.

● No sanitizer was set up in the kitchen area.
