● Food employee(s) were putting on gloves without washing hands prior.

● Food employee(s) were not washing hands in an approved hand-washing sink or automatic hand-washing facility.

● The person in charge was unable to demonstrate proper knowledge of food safety and prevention.

● The person in charge did not ensure that employees are properly sanitizing equipment or utensils. Employees stated the owner takes unclean utensils and dishes to personal home to wash in dish machine.

● A food employee was touching ready-to-eat food with their bare hands.

● Cooked potatoes, sliced tomatoes, and tuna were not properly date marked.

● There was buildup of dirt and debris inside the ice machine.

● Person in charge did not ensure that employees are informed in a verifiable manner of their responsibility to report information about their health as it relates to diseases that are transmissible through food.

● An employee picked a red spoon out of a container with a bare hand.

● There was a dirty ice maker drip edge and container holding the ice scoop.

● There was an air gap between a plumbing fixture and a piece of equipment that was not sufficient to prevent back siphonage of contaminated water.

● A food employee was taking money and then putting gloves on to handle a ready-to-eat item instead of washing hands first.

● There was no date mark on an open container of ham slices to show when the food should be discarded.

● Sanitizing solution in a dishwashing machine did not meet minimum requirements.
